Case Management Services
The Case Management services at Carolinas Specialty Hospital begin at a patient's admission. Each patient is assigned a Case Manager who will meet with the patient and family within the first few days of a patient's stay. They discuss the patient's goals for recovery and discharge. Preparation for discharge begins with that first meeting with the Case Manager.
Each week the Case Manager leads the health care team in a Patient Care Conference. The multi-disciplinary team includes physicians, nurses, respiratory therapists, pharmacists, dietitians, and rehabilitation therapists. Each member of the team discusses the patient's progress toward goals and any barriers to discharge. The Case Manager assists the patient with all aspects of discharge planning including patient and family education, setting up community resources, and arranging for any medical equipment that might be needed at home. If a patient is not able to discharge home, Case Management will assist the patient or family in choosing the appropriate facility to meet their needs.
