If you or your family member would like to be evaluated for admission to Carolinas Specialty Hospital, please call our Admissions Office at 704-304-6430. If you are currently a patient in the hospital, please talk with your physician or Case Manager.
Once your physician has referred you to Carolinas Specialty Hospital, a clinical liaison will visit you in the hospital. She or he can explain the transfer process and answer any questions you may have.
It is our goal to make the admission process as streamline as possible. You or your family will need to provide your insurance card and complete admission paperwork. The admission paperwork is designed to help inform you of your rights as a patient and give our staff your contact information.
Carolinas Specialty Hospital encourages patient and family involvement in the patient's Plan of Care. Our clinical team holds weekly Patient Care Conferences to assess a patient's progress toward their goal. CSH recognizes that each patient is unique; therefore each patient's Plan of Care is customized to meet the needs of the patient.
